Authorization: The patient or a legal representative must sign the required forms. For minors (under 18), this means a custodial parent or guardian, with proof of guardianship. If the patient is unable to sign due to physical or mental incapacity, the responsibility falls to the next-of-kin or a legally appointed guardian, also requiring proof of guardianship and an indication of the patient’s disability.

Special Cases: If you are acting as a Power of Attorney (POA), Healthcare Surrogate, or Healthcare Proxy, it’s essential to provide the relevant legal documents and court orders. Note that such cases might need extra review, potentially extending processing time.

Fees: There may be a fee for copying medical records, whether for personal use or third-party requests, excluding those needed for continuity of care. We assure you that any charges will comply with HIPAA’s guidelines for reasonable costs and adhere to Texas law.
