Streamlined Medical Referrals:
If you have health insurance, we’ll handle
your medical referrals the day before your
visit, ensuring everything is ready with your
insurance. This proactive approach saves time
during your appointment.
Referral Authorization and Insurance:
If your insurance requires a primary care
physician (PCP) referral outside our team, obtain
authorization from your PCP in advance to avoid
delays. We recommend getting at least six visits
per year approved to better serve your needs.
What to Bring:
Please bring your insurance verification and a
copy of your ID to your appointment. Arriving 30
minutes early is recommended.
Accepted Insurances:
We accept most health insurance plans except for
workers’ compensation insurance (Workers’
Comp).
We understand that medical expenses can
be inconvenient. Our clinic offers automated
monthly payments, with fees calculated
according to your income, ensuring we can assist
you as much as possible. You have the flexibility
to choose the withdrawal date from your bank
account, making it convenient for you. We accept
payments with checking accounts and major credit
cards for your ease. Unfortunately, we do not accept Credit Care.

You’ll need to pay for any appointments and treatment at the time of service. Payment via cash, check,
or credit card is accepted.
